For complex histories and a more detailed analysis of previous tests and treatments. If additional review or conversation time is needed, the time and fee will be agreed upon in advance.

Before Your Consultation｜咨询前准备
For both consultation options, please complete the medical history, current concerns, and remote-consultation consent form before your appointment.
两类咨询均须提前填写病史、目前病情及远程咨询同意书。
Please also submit clear photographs of the top and underside of your tongue. Dr. Jin will provide submission instructions when arranging your appointment.
请预先提交清晰的舌面及舌底照片。预约时将告知材料提交方式。
Arrange Your Consultation｜预约流程
- Contact Dr. Jin with a brief description of your concerns and your location.
联系金教授，简述病情并告知所在地区。
- Confirm the consultation option, appointment time, fee, and required materials.
确认咨询类型、时间、费用及所需材料。
- Submit the intake and consent forms, tongue photographs, and any records requested for review.
提交病史与同意书、舌照，以及约定预审的病历资料。
- Complete payment through Zelle using the payment details provided after your appointment is confirmed.
预约确认后，按提供的收款信息通过Zelle付款。
Consultations are available in English and Chinese. The consultation method will be arranged when booking.
